Monday's webinar will speak directly to the changing nutritional needs and resources as a patient moves from the acute care setting to the home setting.
Learning Objectives:
State and explain the differences in malnutrition between the acutely malnourished patient and the chronically ill malnourished patient.
Discuss the changing focus on how malnutrition is addressed through the stages of recovery, including during transitions of care.
Address the recommendations for re-assessing nutrition status through recovery.
