Transitioning complicated nutrition support patients from hospital to home involves coordination between multidisciplinary clinicians, patients, and caregivers. Prescribing a home regimen that is safe, effective, and practical is important to reduce the risk of complications and noncompliance. This session will focus on best practice recommendations to ensure discharge nutrition support regimens are safe and optimized. Clinical and compliance considerations for both adult and pediatric nutrition support patients will be the focus of this webinar, the second in the series.
