Gaps
exist in the continuity of care provided to individuals who are
malnourished. As patients transition from home to hospital and then back
to home or to another facility, lapses in interprofessional nutrition
care occur. This results in lack of improvement or delayed resolution of
malnutrition, prolonged rehabilitation, and increased health care
costs. This webinar will address malnutrition and the transition of care
across various settings.
